4.vessel will call or pass through the following ports enroute to saudi arabia 1...2...3...4...
The undersigned (the owner, agent, captain or co. Of the vessel) accordingly declares that the information provided (in responses shall not call or anchor on any other ports than the A.M. enroute to Saudi Arabia.
Written on the ...day of ...
Signature of vessel’s owner, agent, captain or co.
The declaration (if any) must be notarized by notary public or certified by CCPIT.
The declaration is not required if shipment is effected through national shipping co. Of Saudi Arabia (NSCSA) or united ARAB shipping co. (SAG).

若由沙特阿拉伯国家航运有限公司或阿拉伯联合国家轮船公司承运的话，则无需出具该船证。
