The above cargo was shipped on the above ship by [insert name of shipper] and consigned to [insert name of consignee or party to whose order the bill of lading is made out, as appropriate] for delivery at the port of [insert name of discharge port stated in the bill of lading] but the bill of lading has not arrived and we, [insert name of party requesting delivery], hereby request you to deliver the said cargo to [insert name of party to whom delivery is to be made] (hereinafter “X”) or such party as you believe to be or to represent “X” or to be acting on behalf of “X" at [insert place where delivery is to be made] without production of the original bill of lading.

看起来这是提单丢失后让你们出的保函,大括号里就是要你填写相关的内容:
insert name of shipper: 填发运人名称
insert name of consignee or party to whose order the bill of lading is made out: 填收货人名称(如果提单上有直接收货人),或填指示人名称(如果提单上做的是TO ORDER OF .....)
insert name of discharge port stated in the bill of lading:提单上的卸货港
insert name of party to whom delivery is to be made: 填收货人名称,也就是你要船公司把货放给哪个
insert place where delivery is to be made: 在哪里交货
insert name of party to who will issue the letter of indemnity 出具保函的公司名称
也就是船公司在提单丢失后要你出一个保函,照格式大部分按提单内容填就是

前面一段也是要你填公司名称,签字代表人,地址什么的,盖章.你最好咨询要求你出保函的船公司
