1. Are all of your buildings constructed of materials which resist unlawful entry and protect
against intrusion?
2. Do all of your buildings have adequate locking devices on external and internal doors and
adequate lighting inside and outside your facilities?
3. Are all of your facilities enclosed by fencing?
4. Do you have alarm systems and video camera surveillance at all entrances to your facilities
as well as receiving and loading docks?
5. Do you have 24 hour, 7 day per week security guards on the premises that man or monitor
the gates through which vehicles/personel enter and exit?
6. Do you provide a parking area for private vehicles that is separate from the shipping,
loading dock and cargo areas?
7. Do you perform a seven-point container inspection (as defined by U.S. Customs
and Border Protection) prior to loading a container that ensures the container and locking
mechanisms have not been tampered with?
8. Do you have a procedure in place for affixing, replacing, recording, tracking and verifying seals
on containers?
9. Do you use high security seals that meet or exceed ISO/PAS 17712 standards on all of your
factory loaded containers?
10. Do you have a proper storage area for empty and full containers to prevent unauthorized
access?
11. Do you have procedures in place to properly mark, weigh, count and document products that
you receive/ship and report any discrepancies?
12. Do you conduct employment screening and background checks on prospective employees
and do you have a termination policy to remove indentification, facility and system access?
13. Do you prohibit unauthorized access to your facilities and conveyances?
14. Do you require positive identification and tracking for all visitors, employees, and vendors?
15. Do you have procedures in place to challenge any unauthorized/unidentified persons that
enter your facility?
16. Do you mark and segregate international product?
17. Do you have a designated security officer to supervise the introduction of cargo?
18. Do you have a security awareness training and threat awareness programs for your employees,
that would help in recognizing internal conspiracies, discovering and addressing unauthorized
access, document fraud, and computer security controls?
19. Do you conduct annual self assessments on security control issues as outlined above?
20. Does your automated systems use individually assigned accounts that require a periodic
change of password?
21. Have you undergone a C-TPAT and/or Social Compliance audit within the last twelve months
by any of the following third party auditors such as, SGS, CSCC, Intertek or Bureau Veritas?
( This does not include quality audits only C-TPAT and Social Compliance. )
If yes, please attach a copy of the audit results and check the box to the right.
22. Are you C-TPAT Certified or Validated with US Customs and Border Protection?
