本人是1月提交的CEC申请，已经录了指纹。昨天IRCC发来消息要求我提交后续材料，系统消息里列出的所需要提供的后续材料有三项，如下：
In order for us to continue the processing of your application, we require the following documents:
Schedule A: Please provide a separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. Please note that you must provide details since the age of 18 or the past 10 years. Be sure to not leave any gaps.
The enclosed Supplementary Information Request for Students form. Please complete the form fully with information concerning any current and past studies.
CV/Resume: Please provide a copy of your resume (CV), a document containing a brief description of your education, qualifications and experience.
其中第二项enclosed supplementary information request for students form 是什么呢？
而且在提交材料的页面，只有两个选项，如图所示， 和系统信息所要求的三个补充材料并不匹配。

请问下各位朋友，有没有遇到过这样的情况， 我应该如何上传呢。

第一个是general的材料上传窗口，除了5569第二个，其他整合上传第一个。

有窗口的， 独立上传。

没有独立窗口的。 可以合并上传。

可以把除了5669以外的材料整合成一个文件，然后通过第一个窗口上传，第二个上传5669就可以
