1) Part 1-15, in care of name, is that okay to fill in my name and address here? The reason that I am concerning because I can not find anywhereto type in my address in the entire I 751 form. Usually, a complete form requires applicant's address information. 2) I 751 instruction mentions " answer all the questions fully andaccurately" does that mean I should fill words in all the empty spaces,such as N/A. etc. I am not allowed to leave any space blank. Forexample, Part 5, we have no children, I just typed “N/A” in “1a, Child1, family name” and left all the rest include Child 2, 3 blank. That isincorrect. I should type N/A in all the empty spaces

1. 填写自己的姓名，地址。2. 没有孩子，第一行填写NONE or N/A，下面的可以空着。你如果都想填写N/A也是可以的。
